Job Description
Candidates are required to hold a clear enhanced CRB, references covering the last 5 years and a full CV covering and explaining any gaps in your employment history and also having the reasons for leaving previous posts.
As part of the team and under the general supervision of a senior practitioner or team manager, you will deliver an effective social work service to older adults with Mental Health problems in Essex. Principal accountabilities Undertaking a role in a department of the Mental Health Services by raising or contributing to discussion of issues and proposals and undertaking development work as required. Undertaking assessment of need in line with the requirements of the Care Programme Approach and the Community Care Act. Holding a caseload of, and acting as Care Co-ordinator for, people who have more complex mental health needs, some of whom may also suffer other disabilities, working effectively in a multi-disciplinary setting. Preparing, implementing and monitoring care plans. Working with consumers and their families/carers in both Hospital and Community settings. Participating effectively in Team Meetings. Providing advice/learning support to staff/colleagues. Being aware of current research and initiating local processes in order to improve quality of service. Attending and participating effectively in supervision by the Team Leader/Senior Practitioner, on a regular basis, in accordance with the Directorates policies. Co-working effectively with other professionals including attending multi-disciplinary case discussions. Participate in the Community Mental Health Teams On-Call Rota. Providing a response to referrals and undertaking assessments. Monitoring and preparing reports as required in line with Essex County Council and North Essex Mental Health Partnership NHS Trust policies. Recording information accurately and appropriately in line with Client Access legislation using Information Technology systems and in line with current operational policies and procedures. Working allocated hours in a flexible way (occasionally out of hours if necessary). The post holder will be expected to be committed to continuous workplace learning, which may include undertaking formal training, as part of both individual and organisational development. Any other duties as may reasonably be required. PERSON SPECIFICATION Education and Training DipSW, CQSW, CSS or other equivalent social work qualification. Other alternative recognised qualification as appropriate. Evidence of post qualifying training relevant to the post. Work Experience Experience of inter-agency working. Able to work and a commitment to working in partnership with clients and carers. Evidence of ability to work as part of a team. Evidence of and ability to contribute to the development of services as part of a wider team. Experience of working with other agencies. Experience of managing in a rapidly changing environment. Ability to prioritise own workload and that of others in the face of competing pressures. Skills and Knowledge Ability to communicate clearly, both verbally and in writing. Skills which would contribute to successfully managing change. Ability to empathise with clients and carers needs. Proven ability to undertake Mental Health assessments. Ability to form sound assessments directly and indirectly from knowledge of others work. Ability to advocate on behalf of clients. Strong commitment to rights of people with needs arising from their mental health and a commitment to providing choice and taking the wishes and feelings of individuals into account.
