Job Description
Candidates are required to hold a clear enhanced CRB, references covering the last 5 years and a full CV covering and explaining any gaps in your employment history and also having the reasons for leaving previous posts.
3. Manage a caseload which may include vulnerable users or users with a variety of health problems.
4. Make appropriate assessments of individuals’ social care needs in collaboration with health, social work colleagues and other care agencies, taking into account the wishes of individuals and their carers.
5. Secure the delivery, within the budget, of care plans involving contracts with service providers.
6. Maintain an effective system for co-ordinating, monitoring and reviewing care plans and service standards.
7. Identify gaps in services and advise managers accordingly.
8. Respond to requests for assistance, taking sufficient basic information about the needs in question to determine the level and type of assessment required.
9. Maintain case records and fulfil procedures and statutory duties in line with national and local policies, practices and procedures.
10. Inform the line manager of specific needs which cannot be met because of the unavailability of services or because of inadequate budgetary provision.
11. Work within budgetary limits laid down by Council policy and operate within the Financial Regulations and Standing Orders of the Council.
12. Work flexibly both within a clinical team and within the Department as a whole to meet pressures, including within Fast Response Team and at London Chest Hospital.
13. To participate in regular rotation within team, to over time work in each section of the team.
14. Ensure that assessments pay particular attention to individual strengths and wishes, and that they take into account needs in all relevant user categories.
15. Undertake at all times to take into account the views of users and their carers and facilitate their participation in the assessment process.
16. Negotiate with users, carers and other concerning the methods to meet the needs identified by assessment.
17. Ensure that the care plan includes a clear statement of agreed aims for each service or resource, and clear specifications for each service provider.
18. Assist users and carers to play a full part in the care management process by extending to them advice, support and guidance, which may include counselling, and arranging where necessary access to translation, interpretation or advocacy service.
19. Take responsibility for implementation of the care plan once agreement has been reached.
20. Ensure there is adequate co-ordination of services.
21. Provide information and advice on the resources and services available to adults in social and related services.
22. Provide information to Discharge Planning and case conferences to access the most appropriate service provision for users and their carers.
