Job Description

1. Assist with the effective and efficient provision of complex community care assessments, care planning and care management within the context of multi-disciplinary working.

2. Carry out and co-ordinate multi-disciplinary input into community care assessments, providing statements of needs.

3. Develop and promote the role of social work within the multi-disciplinary team.

4. Carry out care planning for people who have received community care assessments, producing written care plans.

5. Be responsible for monitoring and reviewing care plans.

6. Work with clients, their families and carers providing support practical and emotional and advice, as required.

Candidates are required to hold a clear enhanced CRB, references covering the last 5 years and a full CV covering and explaining any gaps in your employment history and also having the reasons for leaving previous posts.

Candidates need to be registered with the General Social Care Council.
