Job Purpose:

To provide a range of high quality mental health services in the Mental Health Centre and outreach facilities Core Accountabilities.

To work as part of a team to provide mental health services including a range of interventions, skill development and social activities aimed at improving or maintaining optimum mental health.

To contribute to the monitoring, evaluation and development of services To maintain accurate and up to date records of contact with service users.

To liaise with relevant people - particulary the keyworker/care co-ordinator in CMHT as part of the Care Programme Approach To participate in supervision, team meetings and training as required

Candidates are required to hold a clear enhanced CRB, references covering the last 5 years and a full CV covering and explaining any gaps in your employment history and also having the reasons for leaving previous posts.

Candidates need to be registered with the General Social Care Council.
