To undertake person-centred planning in order to maximise the independence of the individual and promote social inclusion

To ensure that the social care needs of individuals are met, within the eligibility policy of the department, by working in partnership with service users, their carers, professionals in other agencies, and service providers.

To establish arrangements and services which make clear and measurable improvements to the quality of life of users and carersExperience in assessing need and planning care with individuals.

Candidates must have experience with adults & older people in a community setting.

Candidates must have experience in working effectively with other agencies and professionals.

Candidates are required to hold a clear enhanced CRB, references covering the last 5 years and a full CV covering and explaining any gaps in your employment history and also having the reasons for leaving previous posts.

Candidates need to be registered with the General Social Care Council.
