SERVICE Adult Physical Disabilities Service Unit
JOB TITLEOccupational Therapist
CURRENT GRADE PO1/PO2
1.MAIN PURPOSE OF JOB
To provide a specialist assessment service for the London Borough's residents of any age with substantial physical disabilities in accordance with relevant legislation and departmental eligibility criteria. In conjunction with the service user formulate a plan to enhance their independence either by rehabilitation, equipment or home adaptations. The needs of the carer will also be carefully considered.

3.RESPONSIBILITIES AND PERSONAL DUTIES
1.To hold a caseload of service users which will be allocated and managed in accordance with departmental procedures.
2.To visit people with substantial disabilities in their own homes and assess their functional needs and environmental difficulties.
3.To fully take into account the needs of carers and other family members.
4.To arrange for the provision of equipment that will meet the service users needs and give instructions on use to both user and carers.
5.To work with other agencies and sections of the Department to plan and recommend adaptations that are necessary and appropriate for the service user. This may include architects and medical consultants.
6.To make written recommendations to the Housing Department to support service users applications for Disabled Facilities Grant.
7.To make written recommendations to Housing Associations for adaptations.
8.To monitor the installation of the adaptations both during and after completion to ensure they meet service users needs.
9.To keep accurate records and case notes of all work carried out, using the Department's policy, procedure and standards on casework recording.
10.To ensure that all Health and Safety requirements are met throughout the implementation of any work plan.
11.To produce reports on the functional needs of service users where appropriate e.g. for rehousing, employment.
12.To respond to and where possible to resolve complaints within the Borough's complaints procedure.
13.To work with other statutory agencies and the voluntary and private sector to facilitate an effective service delivery.
14.To assist with in-house and external training for staff involved in the provision of care for disabled people. This will include childcare staff.
15.To participate in the training and supervision of OT assistants and OT students.
16.To access appropriate assistance where language difficulties present problems in care management.
17.To advise Senior OT about any trends or emergency issues that may impact upon the delivery of service.
18.Any other duties as requested that fall within scale of responsibility.
19. Where required to refer clients to other agencies – both within and external to the council
